The Rediscover Singapore in 57km event is organised by 65 adventure with the support of the ActiveSG and OurSGgrant.

For all event participants, you are required to complete this questionnaire before coming down on your registered date of the event. This form is a declaration of your health and should be completed truthfully. Submit a new form for each individual.

Participants below the age of 18 must complete this form with parents’ consent.

All participants must be fully vaccinated before participating in this programs. Exceptions are made for medically exempted adults and children below the age of 12 years old.
